Life-saving role of heart attack centres confirmed in new study

ScienceDaily (Nov. 13, 2012) ? Recent studies questioning the role of specialist heart attack centres produced misleading results because doctors tend to send the sickest patients to have the best care, according to new research.

Many heart attack patients in the UK are sent to a specialist centre for primary angioplasty -- a surgical procedure to reopen the blocked artery. Randomised trials have found that angioplasty is much more successful than drug treatment alone, but research based on "real-world" data suggest that patients given an angioplasty don't tend to do better.

Now researchers at Imperial College London have shown that the apparent lack of benefit in the clinical records is due to high-risk patients being more likely to be sent to a heart attack centre, which skews the data. After taking this bias is into account, they find that primary angioplasty reduces the death rate from heart attacks by 22 per cent.

They say the findings confirm that heart attack centres play a vital role, and should be made available more widely. The latest figures show that 82 per cent of heart attack patients in England and 30 per cent in Wales receive a primary angioplasty, with wide discrepancies in access between regions.

"There has been some debate in the cardiology community about whether it is worthwhile to run specialist heart attack centres, despite evidence from clinical trials that they save lives," said Dr Iqbal Malik, one of the study's authors, from the National Heart and Lung Institute at Imperial . "This study resolves an important question. We must strive to make sure everyone in the UK has access to the best emergency treatment in the event of a heart attack."

The findings are published today in Circulation Cardiovascular Quality and Outcomes. The authors warn that in real life, doctors faced with a very sick patient tend to give them the most effective possible treatment. This phenomenon -- termed allocation bias -- is good medical practice but can make "comparative effectiveness" research unreliable. Adjusting for this bias is difficult because doctors may base their decisions on many features that are difficult to document. The Imperial team developed a method to help other researchers detect when a disease is vulnerable to this form of bias in evaluation of its treatments.

Co-author Dr Sayan Sen said: "Comparing treatments based on clinical records will always be hindered by the good wisdom of the first-line doctors, who choose the most effective therapy for the most sick patients. We demonstrate that decisions regarding the therapy of heart attack patients should be tested in the most reliable way, namely a randomised trial, and should not rely on registries."

Ellen Mason, Senior Cardiac Nurse at the British Heart Foundation, which co-funded the research, said:

"It is essential to reopen a blocked coronary artery during a heart attack, to prevent part of the heart muscle from dying. A blocked artery can be re-opened during a heart attack by using clot-dissolving drugs or primary angioplasty, where the artery is reopened using a tube. Both these emergency treatments have saved many lives and reduced the amount of long lasting damage to the heart.

"At the moment, throughout the world there is a trend towards using primary angioplasty in more patients -- the UK continues to follow this trend because of continued research showing better outcomes for patients and quicker discharge times. Heart attack centres are essential for providing 24 hour angioplasty, and it's vital that the UK continues to keep up with the latest in heart attack research and treatments."

The researchers also received funding from the Medical Research Council.

And thanks to this game, I believe that students at the Philadelphia Campus have been able to grow closer together.? Someone will eventually take a break and want to play some SoC.? All it takes is one person to say, ?Anyone want to play Settlers??

The game is designed to be played off of both skill and luck.? Each player starts off with two settlements and two roads, and will try to build their communities until they get 10 points.? Each settlement is worth 1 point, each city is worth 2, and so on.? It takes skill to know where to place your initial settlements and where to build future settlements and cities.? But a roll of the dice determines the resources you get to build with.? It can be anyone?s game.

I couldn't find any pictures with a girl playing yet... but trust me, there are.

The games last for over an hour each, so playing multiple games would be out of the question in most circumstances.? But the hilarity that ensues during these longs games is always sure to put smiles on everyone?s face (even for those who watch us play).

'Ghastly mess' at BBC: More quit amid sex scandal

After being accused of covering up former BBC star Jimmy Savile's sexual abuse, the BBC falsely reported that a Margaret Thatcher-era politician had sexually abused children, leading to the resignation of the network's chief and the arrest of a former producer. NBC's Michelle Kosinski reports. ?

LONDON -- The two most senior figures at BBC News ?stepped aside? Monday, as the public broadcaster revealed that the corporation?s outgoing director general will get his full year?s salary of $715,000 despite resigning under pressure after 54 days in the post.

The chairman of the broadcaster's governing body described revelations about the BBC?s multiple missteps in reporting a historic child sex abuse scandal in Britain as a ?ghastly mess? and said the BBC needed a ?radical overhaul.?

"The basis for the BBC's position in this country is the trust that people have in it," Chris Patten, a one-time senior figure in Cameron's Conservative Party and the last British governor of Hong Kong, told the BBC. "If the BBC loses that, it's over."

The widening scandal also had implications on the other side of the Atlantic: Mark Thompson, until recently the man in charge of the organization, takes over as CEO of The New York Times on Monday.

Thompson's successor as Director General, George Entwistle, resigned Saturday -- taking the blame for an editorial blunder in which flagship BBC program ?Newsnight? aired false child sex abuse allegations against a former politician.

On Monday, Helen Boaden, the BBC?s director of news and current affairs, and her deputy Steve Mitchell, ?stepped aside,? the BBC?s media correspondent Torin Douglas reported.

The BBC's press office said it could not yet confirm the report but the BBC said on its own news website that there would be an announcement later in the day.

The BBC faces police and other investigations into claims that hundreds of people, some as young as 12, were sexually abused over the course of decades by one of their top personalities, the late Jimmy Savile.

It is also facing awkward questions over how the same "Newsnight" program chose not to air a report last year that investigated complaints against Savile.?

Payout
The BBC's governing body confirmed that Entwistle would get a payout of $715,000. It said the settlement took into consideration that Entwistle would continue working on BBC business, including two inquiries in the child abuse scandal.

The U.K. government quickly signaled its displeasure at the payout, with minister Maria Miller saying:?"This is a large amount of money, and tough to justify considering the circumstances of Mr. Entwistle's departure."

BBC Director General George Entwistle resigned on Saturday as the BBC spiraled further into scandal over its coverage of two separate sex abuse cases ? one, a cover up, and the other, a possible wrongful accusation. NBC's Keir Simmons reports.

John Whittingdale, chairman of the House of Commons committee on culture, media and sport, said he was surprised by the settlement and has sought an explanation.

?My immediate reaction is that it cannot be justified but I will want to hear exactly why they think it is appropriate. ...?I think almost everybody hearing this news will say 'how can somebody who has had to leave in these circumstances, as a result of a serious failure, nevertheless get a whole year's salary,'??Whittingdale said.

Opposition politician Harriet Harman said the payout ?looked like a reward for failure,? according to a BBC report.

The BBC said Entistle's contract stipulated that he receive six months' salary, but that sum was doubled in order to ensure a speedy departure and transition.

Former minister David Mellor has criticized Entwistle as having the "leadership skills of Winnie the Pooh,"?according to The Telegraph.

Thompson, the new CEO of the New York Times, said he did not know about the nature of the investigation by "Newsnight" into Savile, and had no involvement in the decision to drop the report, which occurred while he was director general.

Thompson did not immediately respond to a request for comment regarding Entwistle's resignation. Earlier, he declined to be interviewed about his plans for the New York Times, Reuters reported.

The BBC, celebrating its 90th anniversary, is affectionately known in Britain as "Auntie," and respected around much of the world.

But with 22,000 staff working at eight national TV channels, 50 radio stations and an extensive Internet operation, critics say it is hampered by a complex and overly bureaucratic and hierarchical management structure.

Rupert Murdoch's Sun tabloid gleefully reported Entwistle's departure with the headline "Bye Bye Chump."

If nothing else, it proves that every so often, the CIA can pull something off - and that yes, Canadians are just about the nicest people on the planet.

The film is a whopper of a tale, one designed for Oscar nominations, Best Picture and Best Director among them.

The details make for a rip-roaring story, which Affleck invests with old-school directorial ?lan.

Ben Affleck leaps on to the A-list of directors with this relentlessly entertaining thriller, combining comedy and nerve-jangling suspense to maximum effect.

Multimillionaire activists, big labor unions and major corporations combined to pump more than $363 million into political fights over 11 propositions on Tuesday?s state ballot, a California Watch analysis shows.

That?s about $20 in political spending for each of California?s 18.2 million registered voters.

By law, state ballot initiatives are exempt from the tough donation limits that otherwise apply in California elections.

In contests over proposed tax increases, car insurance rates, criminal justice reforms and political spending by labor unions, donors with deep pockets took full advantage.

Forty-seven donors ? individuals, companies and political committees ? donated more than $1 million apiece on initiative campaigns, a review of campaign finance data provided by MapLight.org shows.

Seven donors each gave $11 million or more.

The unprecedented spending spree was a sign of just how far the 101-year-old California initiative process has strayed from its origins. In the beginning, initiatives were a Progressive-era reform devised to allow ordinary citizens to sidestep a legislative process controlled by monied special interests.

This year, combined spending over the two most hotly contested ballot measures ? Gov. Jerry Brown?s Proposition 30, which sought a tax increase to fund public education, and Proposition 32, a conservative attempt to bar unions from making political donations ? topped $195 million, with special interest donors paying much of the freight.

Here, from campaign finance data, are some winners and losers in California?s initiative battles:

• Winners: Big Labor

  The state?s unions went all in for their successful effort to stop Prop. 32, the anti-union measure pushed by the Lincoln Club of Orange County and other conservative groups. Unions spent more than $60 million to block it. Much of the union money went to committees also pushing Prop. 30, the successful Brown tax measure. In addition to the California Teachers Association ($32.5 million), big donations came from the Service Employees International Union ($21.6 million) and the California Labor Federation ($6 million).

• Winners: A Facebook Tycoon

  <a href="http://californiawatch.org/dailyreport/former-facebook-executive-funds-anti-trafficking-initiative-16936">Chris Kelly</a>, former chief privacy officer of Facebook and an unsuccessful candidate for state attorney general in 2010, gave $2.3 million to Proposition 35, which would toughen prison sentences for human trafficking. Kelly?s donation was more than 60 percent of the measure?s total budget. It had no formal opposition and won easily.

• Winners: Agroscience

  Proposition 37 would have <a href="http://californiawatch.org/dailyreport/food-companies-fight-genetically-modified-label-measure-18621">required special labels</a> on genetically engineered food. The unsuccessful measure, pushed by the organic food industry, drew multimillion-dollar opposition from Monsanto Co. ($8.1 million) and DuPont ($5.4 million), in addition to Bayer CropScience, BASF Plant Science and Dow AgroSciences ($2 million apiece).

• Winners: The Police Lobby

  The Peace Officers Research Association of California was on the winning side of four measures Tuesday and had only one loss. The association spent $192,000 to oppose the repeal of the death penalty, Proposition 34. Proponents countered with about $8 million, but the measure failed. The group also put $1.5 million into opposing the anti-union Prop. 32, which lost; more than $150,000 to back Prop. 35, the human trafficking measure, which won; and $150,000 to support Prop. 30, Brown?s successful tax measure. The cops' only bad bet: $100,000 to oppose Proposition 36, the measure to reform <a href="http://californiawatch.org/public-safety/calif-voters-consider-changes-three-strikes-law-18627">California?s three strikes law</a>, which was enacted.

• Losers: The Mungers

  The Mungers: The adult children of billionaire Charlie Munger, vice chairman of Warren Buffett?s Berkshire Hathaway, emerged as California's biggest donors ? and losers ? of all. Molly Munger, a Los Angeles lawyer and liberal crusader, donated $44.1 million to her Proposition 38, to raise taxes for public education. The measure lost, while Prop. 30, Brown's competing tax measure, was enacted. Meanwhile, Charles Munger Jr., a Stanford physics professor, donated about $36 million, most of it to defeat Brown?s Prop. 30 tax increase and to boost Prop. 32, which sought to bar labor unions from making political donations. Prop. 32 also lost. By way of contrast, each of the Mungers outspent one of California?s best-funded political operations, the California Teachers Association. It donated $32.5 million to support Brown?s Prop. 30 and defeat Prop. 32.

• Losers: Out-Of-State Corporations

  Thomas F. Steyer, a liberal activist and managing partner of San Francisco?s Farallon Capital Management, was a big winner with his Proposition 39, the measure to collect more income taxes from multistate corporations that do business in California. Steyer donated $29.5 million of the measure?s $31.4 million budget. General Motors, Kimberly-Clark and International Paper put up $45,000 before opposition to the measure collapsed. The state will collect an estimated $1 billion more in taxes as a result.

• Losers: An Insurance Billionaire

  Mercury Insurance founder <a href="http://californiawatch.org/dailyreport/billionaire-insurance-exec-backs-initiative-change-rate-rules-14628">George Joseph</a> spent more than $16 million on Proposition 33, his latest attempt to reframe the state?s auto insurance laws to allow discounts for motorists changing insurance carriers. Opponents, including the <a href="http://californiawatch.org/dailyreport/consumer-group-fighting-insurance-rates-draws-fire-18199">Consumer Watchdog Campaign</a>, said the measure would weaken consumer protections in present insurance law. Opponents spent only about $276,000 on their successful campaign.

• Losers: Americans for Responsible Leadership

  The <a href="http://californiawatch.org/dailyreport/gop-activist-leads-ariz-group-pouring-millions-calif-ballot-fight-18471">Arizona political committee</a> donated $11 million to boost the failed anti-union Prop. 32 and to oppose the governor?s successful tax measure, but those might not be the only losses for the group <a href="http://www.mercurynews.com/elections/ci_21932215?source=inthenews">amid allegations</a> that it illegally laundered the money to conceal its source. Now, investigators are probing how the campaign cash moved to California from a Virginia-based nonprofit, Americans for Job Security, via a third political committee, the Center to Protect Patient Rights. An official with that committee, Sean Noble, is a Republican strategist with reported ties to Charles and David Koch, Kansas-based GOP megadonors. California Attorney General Kamala Harris has vowed to continue investigating the affair.

Coronation Street actor Bill Tarmey, who played Jack Duckworth in the soap, has died.

The actor, who was 71, passed away in Tenerife on Friday morning.

He is survived by wife Alma, son Carl and daughter Sara.

Nigel Pivaro, who played the
Duckworths? layabout son Terry, said: ?I am devastated to learn of the
passing of someone who became a huge part of both my professional and
personal life, who I came to look on as a second father.

?He was a very wise and dignified man who taught me a great deal and whose counsel I came to value greatly.

?He was a lovely man who cared deeply
for his family and friends. We are all rightly bereft at the loss we
will feel knowing that the world will be a little poorer without Bill to
lighten it up with his warm humour.?

Michael Le Vell, who has played
mechanic Kevin Webster in the show for years, said: ?Bill met my dad
briefly when I first joined Coronation Street. And when my dad passed
away Bill became like a surrogate dad to me.

?He and Liz looked after me and got
me through those early days and we?ve been friends ever since. My heart
goes out to Ali and his family. We knew Bill was poorly, but this is
still a very great shock. I am going to miss him very much.?

Tributes also flooded in from the
entertainment world, with This Morning presenter Eamonn Holmes writing:
?RIP Bill Tarmey aka Jack Duckworth. Known him for many years ? always
remembered the small things. Ruth & I getting more like Jack &
Vera. (sic)?

A life-long heavy smoker, Tarmey suffered from ailing health over the years. He had a heart attack in 1976, followed by a stroke in 1977, and underwent a quintuple heart bypass in 1986.

Family comes first: One of the main reasons for Tarmey quitting Coronation Street in 2010 was to look after his son Carl, who is battling a life-threatening brain tumour

Julie Goodyear, who played Bet Lynch,
said: ?This news is so very sad, not just for me but for all his other
friends and of course his wife Ali, and all his family.

?This wonderful man will be sadly missed.?

Catherine
Tyldesley, who plays Eva in show, said: ?So sad to hear that Bill
Tarmey has died. ?legend #RIPBILL. My deepest sympathies to all who knew
him best xxx.?

Speaking about his illness following a
second heart attack in 2002, which saw him fitted with a pacemaker
afterwards, Tarmey said: ?I?m deeply worried.

?I have cheated death so many times I feel like a cat running out of lives.

?I?ve already died twice and the last time they brought me back to life using a medical version of jump leads.?

Tarmey also long refused to give up smoking,
saying in one interview: ?I could make it easier on myself. I could give
up smoking. I could go and sit in a rocking chair.

?But that wouldn?t be me. That would kill me sooner than the old ticker would.?

Tarmey quit Coronation Street in April 2010 to spend more time with his family, after 30 years playing Duckworth.

His decision to leave also came as his son Carl is battling a life-threatening brain tumour.

Tarmey said: ?You would have had to drag me kicking and screaming from the Street.

?It was my life and I loved it. But I needed to be with my family and Carl.?

His final scenes in the show saw Jack dying peacefully in his armchair at No.9 Coronation Street
after attending his 74th birthday party at the Rovers Return pub.

He
said at the time: ?People often ask me if I like Jack Duckworth and I
always reply, ?Like him, I bloody love him!? I have had the most amazing
30 years playing Jack.

?Because of him I have made some fantastic friends and travelled all over the world and I will be sorry to say goodbye to Jack.?

Tarmey was born in Ashton-under-Lyne on April 4 1941, and was
just three years old when his ambulance driver father, William Snr, was
killed by shellfire as he ferried casualties to a field hospital in
Uden, Holland.

Tragic loss: Tarmey began his career as an extra on Corrie before he moved on to his starring role, which netted him ?100,000-a-year

The family grew up with little money
and Tarmey left school to enter the building trade. He married his
childhood sweetheart Alison ? to whom he remained married for 50 years -
and they had two children.

Before joining Coronation Street as an extra in 1977, he
struggled to make ends meet and said one incident involving his young
son brought home to him the importance of money.

The little boy came home upset one day because he had been dropped from his football team.

Feeling the loss: Michelle Collins was seen leaving the set and appeared downcast

Feeling the stain: Catherine Tyldesley was seen dressed in a onsie as she left work. The actress had tweeted her condolences

Tarmey recalled: ?I couldn?t
understand it and went to see the trainer, who told me he had been
dropped because he wouldn?t take goal kicks.

?His football boots were too small, but he was too afraid to tell me because he knew I couldn?t afford a new pair.

?I went home and cried that night. I vowed that I?d never be poor again.?

Despite his TV fame, music remained
Tarmey?s first love and over the years he released a host of cover
versions and big band numbers.
